PATIENT AND VISITOR RESPONSIBILITIES:
When accessing services at Southland District Health Board we ask you to please:
1. inform staff of any changes to your health.
2. for your benefit, be completely honest with health personnel (who will maintain your confidentiality), about any medical condition or circumstances relating to your health, including any alternative therapies/herbal medications you are presently taking.
3. always inform staff when leaving the ward or area you are attending, letting them know of your whereabouts.
4. in consultation with medical and nursing staff, please ensure that you have planned your own support at home and discussed any discharge concerns you may have prior to leaving the hospital.
5. not damage, destroy, deface or remove any article or property belonging to Southland District Health Board.
6. not offer or promise any payment to any health service staff.
7. not use indecent or abusive language or gestures.
8. respect other patients and staff by refraining from behaviour that may cause annoyance, inconvenience and/or lack of consideration.
9. not smoke in facilities or on the hospital grounds of Southland District Health Board.
10. not bring into hospital any alcohol, illicit or non prescribed drugs without prior approval of a Southland District Health Board designated manager.
11. encourage your family and friends to comply with the visiting hours policy.
