Introduction

The Diabetes Team aims to provide high quality care in a timely manner, focused on identified individual needs, and to enable patients to feel confident in working with us to manage their condition and improve their health while providing the knowledge and skills for them to confidently self manage their diabetes

Services delivered by the diabetes specialist nurses and specialist team include:

  • Co-ordinating  the diabetes management and education of acute patients
  • Education of people with diabetes and significant others in the community
  • Providing a supportive role to G.P and practice nurses in diabetes management
  • Education for healthcare professionals

Diabetes Centre Monday to Friday - 8.30am -5pm. Clinics by appointment only.

Phone advice from diabetes specialist nurses - 9am - 4pm.
