About the Team

The secondary care Kaupapa Māori General Health service – Te Ara Hauora is a Māori Liaison service based at Dunedin Hospital and sits within the Southern DHB Māori Health Directorate. The team comprises of a Registered Nurse and Kaiawhina with support provided by the Southern DHB Kaumatua and others.  Te Ara Hauora service is part of a wider Māori Health team that includes Te Huinga Tahi based in Southland Hospital and the secondary care Kaupapa Māori Mental Health Services of Te Oranga Tonu Tanga and Te Korowai Hou Ora.

About the Role

The Kaiawhina will focus on prevention, health promotion, and early intervention and will provide a navigator role to support Māori patients and their whānau in hospital to return back to their home/community and General Practice to improve Māori health outcomes.  The Kaiawhina will work as a key member of the multi-disciplinary team, providing an advocacy and support role for patients and their whānau with decision making and discharge planning.  The Kaiawhina will provide cultural support and advice to other Southern DHB colleagues and work in collaboration with the Southern DHB Home Team, WellSouth Outreach Service, Māori Health Providers and others.

About You

We are seeking a highly motivated individual who is a recognized Māori leader within Māori Health and/or the local Māori community. 

  • You will have knowledge of Maori world view – Whakapapa, Whanau, hapu, iwi.
  • You will be proficient/learning in Te Reo
  • Have strong communication and relationship skills
  • Good community networks including knowledge of Māori Health Providers
  • High level of confidentiality and ability to act with discretion and sensitivity at all times.
  • Have excellent reading, writing and computer literacy.
  • A full NZ Drivers licence.
