Our Internal Medicine Service, Ward 8 Med, is looking for a dynamic and experienced Discharge Coordinator to join the team on a full-time fixed-term basis.

About the Role – About Me

We are committed to improving the flow for our patients, ensuring that they are given the right care in the right place. Working predominately with the older members of our society, you will require an extensive understanding of Aged Residential Care, Needs Assessment, Primary Health Care Services and Community Providers. You will also be required to build relationships with external agencies and health providers, along with families and elderly patients.


We would like a team player with excellent communication skills and a can-do attitude to support and work across Multi-Disciplinary Teams. You will be confident in your judgements, and able to lead activities that improve patient flow such as rapid rounds.

We’re looking for someone who’s experienced and organised, friendly and calm, able to work independently and at the same time genuinely interested in the best outcome for both patient and team. To be considered the candidates must have or demonstrate:


  • Must have current registration with the New Zealand Nursing Council and a current APC, with the right to work in New Zealand.
  • Demonstrated ability to work both autonomously and collaboratively with other professionals.
  • Excellent time management, interpersonal and organisational skills.
  • The ability to be adaptable, flexible and solution focused.
