About the Team

Our Care Coordination Centre is passionate about the community it serves and works effectively, keeping our clients’ best interests at the top of our minds.  You’ll be joining a small, hardworking team and will play an important part in its success.

As a Clinical Coordinator you will support the workforce to deliver high quality health and disability services by providing day to day clinical leadership and coordination within the Invercargill NASC team. The role will provide advice to and collaborate with the operational and professional /nursing leaders and work in partnership to promote patient safety, efficiency and productivity.  

In conjunction with clinical co-ordination the role will also incorporate a clinical needs assessment function. You will utilise the national assessment tool interRAI to assess and determine best outcomes for our elderly clients. Your role as a CNA will be to assess health of older people clients and if long term supports are required to create a plan of care that is client centred and goal focused, based on the restorative model of care. You will work business hours, Monday to Friday.

About You

We’re looking for someone who is self-motivated, confident and proactive in identifying areas of service development and providing leadership to teams.  It’s essential you’re a Health professional who holds current registration.  Our ideal candidate will have proven experience of at least 5 years in clinical practice demonstrate a high level of competence in verbal communication, assessment and facilitation of services for people who require community based treatment and support services.  You will have excellent problem solving skills and a ‘can do’ attitude, showing an ability to consistently apply access and entry criteria without sacrificing innovation or creativity. It’s important you’re confident with computers and have experience with data entry, Outlook and other computer based systems.  Current experience with the interRAI assessment tool is essential.  Training and ongoing support in the use of interRAI is provided.
