Early Discharge and Rehabilitation Service
- Permanent, part-time position (0.5TE) 40 hours per fortnight
Southern District Health Board (Southern DHB) exists to support everyone across our district to live well, and access the right care when they need it, by delivering high quality, patient-centred and equitable health services to our diverse communities.
The EDRS service currently operates from Monday to Friday 8 – 5pm. This permanent part time position will require you to work 40 hours per fortnight.
About the opportunity
This is an exciting opportunity for a Registered Nurse to join our interdisciplinary team
The primary purpose of this position is to support patients to continue their rehabilitation within their own homes You will monitor their recovery and ensure each individual receives appropriate rehabilitation and/or packages of care at the right time to maximise their functional ability, and maintain their independence in their own home.
A genuine passion for working in the community is essential to the success of this position.
Within this position you will require;
- Advanced comprehensive clinical assessment and problem solving skills
- Ability to work autonomously and collaboratively within the interdisciplinary team
- Ability to direct and delegate to Rehabilitation Assistants
- Excellent communication skills to liaise with health professionals, both within the in-patient services and in the community setting.
- Recent experience of working with the older adults in acute, rehabilitation or community settings
With the growing focus on community based care this is a fantastic opportunity for a Registered Nurse with at least 2 years’ experience.
