We are looking for a flexible and motivated team member to work within our Clinical Needs Assessment (CNA) team.
About the opportunity
You will utilise the national assessment tool interRAI to assess and determine best outcomes for our client group. Your role as a CNA will be to assess health of older people clients and clients under the age of 65 with long term chronic conditions. If long term supports are required you will create a plan of care that is client centred and goal focused, based on the restorative model of care. You will work business hours, Monday to Friday.
You will need to be a registered Health Professional under the Health Practitioners Assurance Act or Social Work Registration Act, a current practising certificate and have excellent interviewing, assessment and problem solving skills.
Ideally you will have experience working with older people in either a hospital, community or aged care environment. You will need an excellent knowledge of community resources and services that are available to the older person.
Previous experience with the use of interRAI is preferred but not essential. Training in interRAI is available and on-going support in the use of interRAI is provided. It is essential that you have a current NZ driver licence.
You will also need a high level of verbal and written communication skills and experience working with electronic systems and be confident using a computer.
