Early Discharge and Rehabilitation Service for the Older Person

Fixed term, Part time (0.7FTE)


The Early Discharge and Rehabilitation Service for the Older Person (EDRS) are looking for an experienced Registered Nurse to join their supportive, enthusiastic nurse led Early Supported Discharge team on a fixed term basis.  The role will see you working collaboratively within an interdisciplinary team providing home-based holistic assessment and treatment for newly discharged older adults within their own homes. 


Within this position you will require:

  • Advanced comprehensive clinical assessment and problem solving skills
  • Ability to direct and delegate to the Enrolled Nurse and Rehabilitation Assistants
  • Excellent communication skills to liaise with health professionals, both inpatient and in the community
  • Recent experience of working with the older adults in acute or community settings


With the growing focus on community based care this is a fantastic opportunity for a Registered Nurse with at least 2 – 3 years’ experience. Your strong skill set and knowledge base will be complimented by your willingness to learn and share your skills with the wider team.  Flexibility is essential as the work is challenging and varied. You will have the benefit of working both autonomously and within a supportive, interdisciplinary team.


The Registered Nurses within the EDRS service currently cover Monday – Friday 9.30 am – 6 pm and 8.30 am – 5pm on Saturdays. This fixed term part time position will require you to work 7 days per fortnight.
