Clinical Nurse Specialist – Long Term Conditions
Our Long Term Conditions service at Southland Hospital is looking for a full time permanent Long Term Conditions Clinical Nurse Specialist based at Southland Hospital. Our team is committed to providing expert clinical care and dedicated support to the patients in our care with long term conditions
In this position you will provide clinical leadership and expertise within this small interdisciplinary team of nurses and doctors.
To be successful in this role you will have a genuine interest in working with patients with ongoing Long Term Conditions and have an understanding of the importance of this model of care. You will need to demonstrate advanced clinical assessment and diagnostic reasoning skills in addition to your excellent clinical assessment and communication skills. Time management and organisational skills are a must as will be your ability to work both autonomously and within a team environment. This is a varied role where you will be seen as a practitioner, educator, consultant, researcher, change agent and leader in the pursuit of clinical excellence and improved health outcomes.
As a senior registered nurse, with a minimum of three years post registration experience preferably in respiratory nursing, you will have the maturity and experience to step in to this role and make it your own. To be considered for this role you will hold a current APC. You will have/ or be working towards a Level 4 PDRP, and a post graduate qualification relevant to this clinical area.
The successful applicant must have a current clean drivers licence with the ability to travel throughout the region as required. The hours of work will be five days per week from 0800 to 1630 but there is some flexibility and this can be discussed at the interview.
