Discharge Coordinator – Medical Ward

Our Internal Medicine Service, Ward 8 Med, is looking for a Discharge Coordinator to join the team. This is a permanent position working 40 hours per week Monday to Friday.
We are interested in hearing from those with proven experience working within an acute setting along with an extensive understanding of community services. We believe our Service is compassionate and friendly and we are committed to high standards of practice. We are focused in improving the flow for our patients ensuring that they are provided with care in the right place. Working predominately with Older People you will require an extensive understanding of Aged Residential Care, Needs Assessment, Primary Health Care Services, and Community Providers. We would like a team player with excellent communication skills and a can-do attitude to support and work across Multi-Disciplinary Teams. You will be confident and able to lead activities that improve patient flow such as rapid rounds.
You will be required to build relationships with external agencies and health providers along with families and elderly patients. You need to be hardworking, self-motivated and be extremely organised to be successful in this role.
