Care Coordination Centre

Dunedin

  • Full-time 1 FTE (40 hours per fortnight)

 

The Care Coordination Centre (CCC) is looking for a Team Leader, to support the Clinical Needs Assessment Team in providing excellence in clinical practice. The Team Leader will be responsible for ensuring effective day to day coordination of work to ensure responsive service provision. The role will also provide triage/duty support to enable better work flow. You will provide efficient, effective and quality leadership to the team who are strongly focused on providing high quality clinical needs assessment services.

 

The Team Leader will report to the Unit Manager and will be strongly supported by the Service Manager and Directorate Leadership. This is a demanding and challenging role where the successful applicant will demonstrate sound professional judgement, strong leadership skills and a clear commitment to high quality care and the development of the Service.  The successful candidate will have a desire and ability to develop a new role and support a very experienced team. You will have experience working with older people and their whanau/family members and be able to demonstrate good clinical decision making, be able to think outside the square and have excellent communication and engagement skills. You will need an excellent knowledge of community resources and services that are available to the older person.

 

Your role will also involve clinical needs assessment where you will assess clients in a variety of health settings using the interRAI assessment tool and create a plan of care that is client centred and goal focused including developing support packages in a holistic and culturally appropriate manner for older people who require long term support in the community.

 

You’ll be working with older people and their family/whanau to achieve the best outcomes for them.  You will need a high level of verbal and written communication skills and experience working with electronic systems and be confident using a computer.

 

You will need to be a registered Health Professional under the Health Practitioners Assurance Act or Social Work Registration Act, a current practising certificate and have excellent interviewing, assessment and problem solving skills.
